Design Required for a Freelance PHP Developer

I'm a freelance web developer, and I require a designer to create a website template that I can use to showcase my work.
I code in PHP/HTML/CSS/JQuery and so I would only need someone to provide me with a design in a PSD file.
I've no set theme or requirements, I just need something modern, clean and relevant.
Please show me any example work you have created, and tell me what sort of design you think would be suitable (possibly using example sites).
Also, provide me with your rates.

A project is typically a fixed-price piece of work, based on specific results, and often completed offsite using the freelancer's own tools.
